The luncheon cost is $50.00 per person. The luncheon & awards ceremony will immediately follow the morning symposium. Our plenary speaker is Nikos Avlonas, Founder & President of the Centre for Sustainability and Excellence.

If paying by check or cash, you will receive an invoice stating the total price that you owe after submitting this form.
To ensure a smooth registration, it is recommended that you pay in advance. Register by

A small block of rooms at the McDonald’s Hyatt Lodge Hotel in Oak Brook has been set up. The special room rate is $159 per night. To make a reservation please call the Hyatt Lodge at call 630.581.6603 and when making a reservation please mention the “Governor’s Award Ceremony Room Block”. Rooms may sell out, we cannot guarantee that rooms will be available.
Registration Deadline and Refunds
The cut-off date for registration, cancellations and registration refunds is close of business November 2, 2012. If you do not notify us by November 2, 2012 of your cancellation, a refund will not be processed and you will be invoiced for any unpaid registration fees. Substitutions can be made at any time, to include the day of the luncheon.
